Making Appointmens:

We see patients by appointment Monday through Thursday from 8am to 5:30pm, Fridays 8am to noon. For Highland Avenue Office call (909) 881-1683 for Fontana Office call (909) 428-3457 for Rialto Office call (909) 546-1270.

Our receptionists' are available daily to help you. Initial examination may take 30-45 minutes, so please allow ample time.

We try our best to stay on our schedule, although emergencies sometimes arise. If we are seriously delayed, we will try to notify you beforehand. Please assist us by being on time for your appointment. Occasionally we may call patients in the waiting room out of turn, if they are urgent or postoperative exams. We ask for your patience if you have to wait, we know your time is valuable.

Your Appointment:

On your first visit, we will begin a permanent record. We will ask you to complete a medical history form, and a brief patient information form. Please bring your insurance information. Try to arrive at least 30 minutes early to your first visit. If you have been under the care of another doctor, we may ask your permission to transfer some of your records.

Cancelling an Appointment:

If you are unable to keep your appointment or are goint to be late, please call our office as soon as possible. This courtesy allows us to be of service to other patients.
